Waist-to-hip ratio is simply the circumference of your waist divided by the circumference of your hips. Because it is a ratio of two lengths, the units cancel and it comes out the same whether you measure in centimetres or inches. Its value lies in what it reveals about fat distribution: a higher ratio means proportionally more fat is stored around the abdomen rather than the hips and thighs. This matters because abdominal (visceral) fat, which packs around the internal organs, is far more metabolically harmful than fat carried lower down. The World Health Organization uses thresholds of 0.90 for men and 0.85 for women, above which the risk of heart disease, type 2 diabetes, and other conditions rises sharply, regardless of a person's overall weight or BMI.

Worked example

A man with a 90 cm waist and 100 cm hips has a ratio of 90 ÷ 100 = 0.90, which sits right at the threshold the WHO classes as elevated risk for men. A woman with the same measurements would have the same 0.90 ratio, but for women the high-risk line is 0.85, so it would fall further into the elevated range.

Why where you carry fat matters

Two people can weigh the same and have the same BMI yet face very different health risks, and a large part of the reason is fat distribution. Fat stored around the hips and thighs — the 'pear' shape — behaves quite differently from fat stored around the abdomen — the 'apple' shape. Abdominal fat includes visceral fat, which surrounds the liver, pancreas, and intestines and is metabolically active in harmful ways, releasing fatty acids and inflammatory signals that disrupt insulin sensitivity and raise cardiovascular risk.

Waist-to-hip ratio is a simple proxy for this distribution. A high ratio flags an apple shape with more central fat, which research consistently links to heart disease, stroke, and type 2 diabetes — often more strongly than BMI does.
